The title clearly identifies the proposed project. It is recommended for the title to be less than 12 words.

Identify your organization and its credibility to provide park projects

Consider the following questions as you prepare this section:

What problem or need does your project address?

What evidence can you provide of this problem or need? Consider community input, assessment data, or health and safety concerns.

What are the conditions in the area adjacent to the project site?



What are the expected measurable outcome(s) that this project will produce?

Consider the following questions as you prepare this section:

Who will benefit from this project?

What improvements will result from this project?

How will the project affect maintenance, budget, or staffing?

How will you track these outcomes?



Describe the methods to be utilized to achieve the desired outcome(s)

Consider the following questions as you prepare this section:

How will the work be performed and who will perform it?

How and when will you engage the community?

What makes this location the most appropriate for this project?

How does the project timing affect the desired outcomes?

How does the project location affect the desired outcomes?



Summary
2 to 3 sentences summarizing the proposed project. The sentences should be concise and allow for a clear understanding of the proposed project.

Does this project require a Joint/Shared Use Agreement?
A joint/shared used agreement is a document that describes the shared management of facilities, land, utilities, programs, or other common elements between two or more parties.

CEQA Lead Agency Name
When more than one public agency is involved, a "Lead Agency" is the public agency that has the primary responsibility for approving a project that may have a significant impact upon the environment.

CEQA Responsible Agency Name
A "Responsible Agency" is any public agency, other than the lead agency, which has the responsibility for approving the project where more than one public agency is involved.

CEQA Document
Complete CEQA document is required.

Adverse site conditions?
Examples of adverse site conditions include but are not limited to, contamination issues relating to soil and water, and fire issues.

Supervisorial District
Visit the LAvote website. Select the dropdown and select "District Map Look Up by Address". Input the park address.

State Senate District
Refer to Supervisorial District Help information.

State Assembly District

U.S. Congressional District
Refer to Supervisorial District Help information.

Park ID
RPOSD uses the California Protected Areas Database (CPAD) Unit ID for a Park ID. To find a Park ID go to https://bit.ly/2Vids3W and search for the park location. Once the park is identified, find the Unit ID number, and use this number as the Park ID.

Project Vicinity Map
A map showing the project's location including its general vicinity. The map shall be detailed enough to allow someone unfamiliar with the area to locate the project.

Site Plan
Depiction of the site(s) to be developed and the specifics of the development activity. The Site Plan must indicate the location of all facilities, other structures, landscaping, and other developments proposed for the project.

Grant Boundary Map
A map that shows the project's general area and delineates the boundaries of grant funded property, inclusive of reasonable public access such as roads and walkways
