Who attended the application workshop:
All applicants eligible for funding are required to attend an application workshop before applying. The workshop may be in-person or via a webinar hosted by RPOSD. Confirmation of attendance will be verified by RPOSD. Whoever fills out this application form should be an agency representative who attended the workshop and must be an added user within the Grants Management System. Date of workshop:
Enter the date of attendance at the workshop, or the date of viewing the webinar.

Type of workshop attended:
Attendance at an in-person workshop will be verified by RPOSD. Webinars can be viewed live or on the RPOSD website at any time.

Our agency/organization has reviewed the requirements; we confirm our current Good Standing status and agree to remain in Good Standing with RPOSD.
A grantee is considered in Good Standing status if in compliance with all requirements of RPOSD Grant Guidelines and grant agreements. For detailed information about Good Standing, please refer to the Section 1.7, Section 3.4.7 of the Measure A Grants Management Manual.

Our agency/organization has read and reviewed the RPOSD signage and branding requirements.
All grant funded projects must include a sign at a prominent location on the project site acknowledging the assistance of RPOSD. The cost of permanent signage development is reimbursable through the grant. Acknowledgement of the assistance provided by RPOSD must also be included in all written materials produced for grant funded programs. RPOSD will provide electronic samples of its graphics for the grantee to use in signage development. Refer to Project Requirements stated under the appropriate grant funding program for more details.

Our agency/organization has read and reviewed the Project Delivery: Grant Closing and Post Closing obligation requirements.
All grantees are required to follow RPOSD procedures to close the grant after completion of the project. For detailed information about Project Delivery, please refer to Section 3.4.5 on the Measure A Grants Administration Manual.

Our agency/organization has read and reviewed the insurance requirements.
All grant funded projects require grantees to meet insurance requirements associated with the grant agreement. Click here for more information.

Our agency/organization understands that we may be required to record a Deed Restriction for this project.
RPOSD requires, to the maximum extent feasible, that the grantee shall cause to be recorded on the title of any real property acquired and/or developed with funds from RPOSD, a deed restriction requiring compliance with the grant agreement, in perpetuity (Section 3.4.5).

Our agency/organization has reviewed the terms and conditions of the Grant Agreement and is prepared to execute the Grant Agreement upon approval of this application.
RPOSD requires applicants to review the grant agreement. The grant agreement is available for review on the RPOSD website.
