Select the type of agency or organization applying for grant funds.

Park inventory last updated on:
This box would only appear if you are a public agency. Enter the date that your agency last completed their park inventory update to the Parks Need Assessment website. Agencies must complete this information on an annual basis to remain in Good Standing(add link here)with RPOSD.

Authorized Representative
The Authorized Representative is the individual approved to make organizational decisions for your agency. This individual should already be an added user within the Grants Management System.

Authorized Designee
The Authorized Designee is the individual approved to make organizational decisions for your agency on behalf of the Authorized Representative. This individual should already be an added user in the Grants Management System. Read the Quick Start Guide for enrollment instruction.

Organization Chart
Upload a document reflecting the operating structure, including the number of staff and staff roles, dedicated towards grant or project administration in your agency. DO NOT upload the organization chart for the entire agency. This information will contribute to the Organizational Capacity Review, as defined on the Measure A Grants Administration Manual.

Has your agency provided RPOSD a Proof of Jurisdiction Support?
Inform us whether a proof of jurisdiction support that applies to RPOSD grant projects has already been submitted to avoid unnecessary duplicate documents associated with your agency. Proof of Jurisdiction Support can be in the form of a resolution or a jurisdiction agreement. Affirmations of submitted document will be verified by RPOSD (Section 3.2.4).
